The folder can also serve as your new gateway to online services such as booking appointments at your health center or checking your status on waiting lists.

It also provides you with useful links to self-care advice, as well as access to our search engines for healthcare centers and pharmacies open outside regular hours.

The Personal Health Folder is a result of the digital information generated by the centers where you receive care, along with any additional information you deem necessary to complete your medical history.

The application includes a specific section on the COVID-19 coronavirus.

What do I need to access My Health Folder?

• Be 16 years or older
• You must go to your health center and make the request in person, providing identification.
• Have an email address and a mobile phone to receive SMS messages.
• If you already have credentials because you previously applied to access the Personal Health Folder website, you do not need to apply again.
